MBI Tool for Patients
These questions are designed to help people with maternal birth injuries find the help they need. They may be unaware that treatment is available for their symptoms, or be uncomfortable discussing these issues with their current health provider.
 
Share these versions in places where patients may see them such as your clinic greeting area, community organisations, notice boards, etc.

Add your branding to the MBI Screening Tool
The terms of use for the MBI Screening Tool allow you to add your own details to the co-branded versions.
  • Use your favourite app (eg Word, Google Docs, Canva or similar) to create a design with your details (eg call to action, logo, contact information, QR code, etc). You will need a portrait version for the infographic and a landscape version for the table format.
  • Save that design as an image (eg take a screenshot or download it and save as .jpg or .png file).
  • Open the MBI Screening Tool in your favourite .pdf reader (eg Foxit, Acrobat or similar). Copy and Paste the image you saved above into the space provided. Re-size the pasted image for best fit. Save the new version with a different file name somewhere you can find it. Your finished version should look something like the samples below.
  • Print your co-branded MBI Screening Tool on good quality paper stock. Laminate it for durability. And share it far and wide.
